Equipment Return

The monitoring system prescribed by your doctor is an FDA regulated medical device, and it is on loan to you. You must return it to ReactDx promptly upon completion of your procedure. A postage-paid return mailer is included in the monitor kit for your convenience.

1. Place all monitoring equipment and any unused accessories back into the monitor kit
2. Place the kit in the postage-paid return mailer (included in the kit) and seal it
3. Place the mailer in your mailbox or any USPS dropoff location for pickup
